You CANNOT Use This For for any of the following

Any type of violent crime, including: personal assault, kidnapping, robbery, etc.
Any type of sex crime, including: rape, sexual assault, etc.
Any type of stolen vehicle, including: trucks, cars, trailers, motorcycles, etc.
Any type of traffic accident, whether there was an injury or not.
To report violent crimes or sex crimes, please call (413) 323-6685.

To report:

Missing persons, stolen vehicles or stolen license plates, you must come into the station.
Traffic accidents involving an injury, please call (413) 323-6685.
To report traffic accidents NOT involving an injury, please do one of the following:

If an officer has responded to your non-injury accident and has said that we cannot take your report because it doesn't meet minimum reporting requirements, you must report your accident to:
Massachusetts RMV
Police Department where it occurred
Your Insurance Company

If an officer has never responded to your non-injury accident, you need to come to our station for assistance in what to do next to report your accident.

You may also call (413) 323-6685 for more information. In either case you may use this form if there was property damage over $500.

You can use this form to report the following crimes

Vandalism
Thefts from a vehicle
Lost property
Burglary of a house, garage or business
Petty theft
Fraudulent use of lost or stolen credit cards, checks, etc

 

This is NOT a fast-response system. All reports will be reviewed and a response will be initiated. You will be advised if a case number is assigned to your incident. This may take up to seven days from the time that you file this report.

If you need a faster response than that, you can come to the police station located on 70 State St and file a report at the Public Counter.

IMPORTANT NOTE:
Knowingly submitting a false Crime Report is a misdemeanor under the Massachusetts State Laws
Chapter 269 Section 13A (and worth up to a year in jail) !! "Don't even THINK about it"
