Make sure you have everything on this list before submitting your application! It will save you time and money, because the Planning Board will not act favorably until all the information they want is in your file.
q Form C entirely filled out.
q Original signatures of all applicants, all owners, engineer.
o If corporation, partnership, etc., proof of authority to sign.
q Abutter list – CERTIFIED – from the assessors’ office. If another town is included, that list must be certified by that town’s assessors’ office.
q Fee according to §270-67.
q Agreements signed by the applicant to pay for the town’s engineering review and the public hearing notice (Form I and Form J).
q The correct amount of plans - §270-21
o 10 copies to the Planning Board.
o 2 copies directly to the Board of Health. The applicant must file these with the Board of Health according to MGL c.41, §81U.
q Full drainage and storm water run-off control report – 5 copies.
q Order of Conditions from the Conservation Commission, if available.
q Letter of Good Standing from the Belchertown Tax Collector on behalf of the applicant and the owner of the property, if different from the applicant.
q Cost estimate, Form G, signed and stamped by the applicant’s engineer.
